Terms of Service
Last updated 1 September 2026
These terms govern your use of My Feedback Form at myfeedbackform.app. By signing in, publishing a form, or answering somebody else’s form, you agree to them. If you do not, please do not use the service.
Who you are agreeing with
My Feedback Form is operated by Myfeedbackform, a company registered in Germany (“we”, “us”). You can reach us at privacy@myfeedbackform.app.
What the service does
You pick a role, adjust a set of questions, and publish a form. We give you a link. Anyone holding that link can answer without signing in, and you read the results. That is the whole product. We may add to it, change it, or take parts of it away.
Your account
- You sign in through Google or GitHub. Keeping that account secure is your responsibility, and anything done through your session is treated as done by you.
- You must be at least 16 years old and give accurate information about yourself.
- An account is for one person. Do not share your sign-in with somebody else.
Share links
A published form is reachable by anyone who has its link — there is no second check on who is answering. That is deliberate: it is what lets colleagues respond without an account. It also means the link is the only thing standing between your round and the open internet. Send it to the people you actually want feedback from, and close the round when you are finished collecting.
What you may not do
- Use the service to harass, threaten, defame or abuse anybody, or to submit unlawful content.
- Try to work out who wrote an anonymous response by technical means, or pressure colleagues into revealing what they wrote.
- Publish a form that solicits special category data — health, religion, sexual orientation, political opinion, trade union membership — or that collects feedback about a person without their knowledge.
- Submit responses automatically, stuff a round with fabricated answers, or otherwise misrepresent how many people replied.
- Probe, scrape, overload or attempt to gain unauthorised access to the service or to another user’s data.
- Resell the service or present it as your own.
Content
The questions you write and the answers people give remain theirs. You grant us the limited licence needed to store, process and display that content in order to run the service for you — nothing more. We do not use it to advertise, we do not publish it, and we do not train models on it.
As a form owner you are responsible for what you do with the feedback you receive. Reading it is the point; using it to retaliate against a colleague is not, and is grounds for us to close your account.
Anonymity is a design, not a guarantee
We do not record who submitted which response. We cannot promise that an author is unidentifiable, because a small round and a distinctive voice can give somebody away without any help from us. Respondents should write with that in mind. Owners should not try to close the gap.
Availability
The service is provided as it is, with no promise of a particular uptime, and we may change, suspend or discontinue any part of it. We keep backups and take reasonable care, but you should not treat this as the only copy of anything that matters to you.
Suspension and closing your account
You can ask us to close your account at any time by writing to us, and everything belonging to it is deleted as described in the privacy policy. We may suspend or close an account that breaks these terms, or where we are required to do so by law — for anything short of serious or repeated abuse we will tell you why first and give you a chance to put it right.
Liability
We are liable without limitation for damage caused intentionally or by gross negligence, for injury to life, body or health, under the German Product Liability Act, and to the extent we have given a guarantee. We are also liable for the negligent breach of an obligation whose fulfilment is essential to this agreement and on which you may reasonably rely, but that liability is limited to the damage typically foreseeable for this kind of agreement. All other liability is excluded. Nothing here limits any right you have as a consumer that cannot be limited by contract.
Changes to these terms
We may update these terms. The date at the top changes with them, and material changes are sent to account holders by email before they take effect. Continuing to use the service after that means you accept the new version.
Governing law
These terms are governed by the law of the Federal Republic of Germany, excluding its conflict-of-law rules and the UN Convention on Contracts for the International Sale of Goods. If you are a consumer, this does not deprive you of the protection of the mandatory law of the country where you live, and you may bring proceedings in the courts there.
